My Hospital Internship Days


Those who are pursuing their Bachelor of Pharmacy (Professional) degree must undergo a hospital internship of 600 hours. According to our syllabus, hospital internship is a mandatory 3-credit coursework which we have to do in 4th year 2nd semester. So, I decided to complete my internship at Trauma Medical Centre, one of the renowned hospital in Cumilla, with my group. Our group consist of six members, four boys and two girls. We are Jahid, Rakib, Jihan, Tithi, Orme, Jobayer.

 

Hospital Internship Day-1 (01-05-2024)

Today I have started my hospital internship at Trauma Medical Centre, Cumilla. The manager is a very friendly person. He provided us many valuable life lesson before starting the work. We visited the admission center, cash counter, information center.


At first, we started visiting admission center where they admit the inpatient in cabin, ward, ICU, CCU, NICU, PICU etc. For providing financial flexibility, they offer different type of facility. This hospital uses high quality software for hospital management. I investigated the admission form and find out the required information.

The admission fee in this hospital is 500 taka. The admission center is less busy, because every patient don’t need to admit as inpatient. The front desk officer Arif sir was very helpful. He provided all the information about admission center and then cash counter, hotline, 2nd and 3rd floor. Although I knew the about ICU, but I was not familiar with CCU, NICU, PICU. He provides us information related to these.

The cash counter is a very busy place. So, two persons are employed here. They take the money from the patient’s relative for different test and provide money recite. In case of inpatient, patient can get service without paying money immediately by making a due against the patient’s ID. For these tasks, they use medical management which is very effective. It can easily calculate the discount, cash received, due etc.

Information and customer center provide information about the doctors, their schedule, radiology and imaging system, fee etc. People can take information through mobile phone also.

With the help of hospital service manager H.M. Taizur Rob Miah Sir, we met with the medical director of this hospital. He was an old person. He asked us our name and hometown, then tell us to meet him if we need anything.

With the help of Arif sir, we came to know that this hospital has mammography which is not available in any other hospital in Cumilla. Moreover, the specialist doctors are also not available. So, the reports have to send Dhaka to get observation. We say other radiology and imaging system from outside. I will tell you about them in future.

Finally, we visited all floors of building no.2 which is a 14 stroyed building. I notable thing is, the fire management system seems very good here and the lift system is well enough to provide facility to the patients, specially who get their word in the higher level floor.

As it is one of the renowned hospitals in Cumilla city, the number of patients is huge. It will be better to increase the number of seats in the waiting of the ground floor. Because, during rush hour, some patients or their relatives didn’t able to seat.

 

Hospital Internship Day-2 (02/05/2024)

Customer dealing is very important part of any hospital. Today we spend time with the hospital employees in information & customer care center and cash counter, see how they deal people. We noticed cash counter has to face a huge workload. In the morning shift, from 10 am to 1 pm, we noticed two or three persons working continuously.

They have to face handwriting prescription problem. I notice two prescriptions where the doctors writing was worse. So, they had to straggle to understand which pathological, radiology and imaging test they suggest.

Some doctors asking for a discount for their patients, some people pressurize to reduce their fee, some people behave rude. They have to deal with all of these as well as take quick action. A number of tests are available in this hospital. Employees must know which tests the doctor suggest. Using digital software help them a lot. Typing speed should be a mandatory skill for them. Everyday at 3 pm and 7 pm, they have to report the authority about their collection and send money with proper documentation.

Information and customer care face workload mostly in the morning. Patients call them for serial of appointment or knowing doctor related information. Some people may behave rude. They seek early appointment to the doctors. In-charge of this, Pinki mam, is very kind and supportive. She provide us two prospectus where we get all the doctors list and radiology & imaging test list. This is very helpful for us.

Our university is now closed because of clash between vice chancellor and teacher’s association. But because of internship, we don’t feel us victim. All of my batchmates are now in Cumilla city and they are doing internship in different hospitals which are near to each other. After completing our tasks, we visited Gomati Hospital where another group of our batchmates are doing internship. My friend Shohan, who is doing internship in CD Path Hospital, also join with us. Together, we spent a good time. My friend Dipu, treat us with sweet from a bakery on the ground floor of their hospital. I noticed that, the cabins of this hospital and it’s environment is very clean and decorated.

Our senior brother Mostafa vai works here as a hospital pharmacist. But unfortunately, he was absent today. So, we couldn’t meet him.


Hospital Internship Day-3 (05/05/2024)

Sample collection is an important part of pathological test. For diagnosis disease, blood, urine, tissue etc. need to be collected. Today we spent the hole day in sample collection point of the hospital and noticed so many things. It is in the 1st floor of building no 2.

Sample collection is a challenging job. The baby and old patients fear of syringe and needle. Most of the case, the babies cry and unwilling to help with sample collector. Sometimes, it is difficult to find out the vein. Some patients even complain about the amount of blood collected. Moreover, the sample collection point has to work with the pathology lab (7th floor). But we wonder seeing that Prya Datta mam easily handle all of these situations.

She explained different types of tube for sample collection, their properties, working procedure etc. She took some sample from patients. Interesting part is, she asked us to collect sample. At first, we fear of this. But with her and two other mam’s supervision, we successfully collected some samples and documented them properly.

Many hospitals use traditional tube for sample collection. Working with these is little bit difficult for both the patients and sample collector. But this hospital uses modern vacuum tube. It is very convenient. This is for the first time I came to know about vacuum tube technology. Moreover, heparin or other anticlotting agents are already presented there. So, we don’t need to add them. I am grateful to those who apply their intelligence for making such type of new technologies and make the patients and technologists life easier. According to me, anyone with two days training can be a skilled sample collector. Because, the basic of sample collection is not that much hard.

Today, for the first time, we got the opportunity to work with patients directly. I believe that hospital is the place from where we can gain experience, we can know about the misery of life, we get the opportunity to talk with people and relief their pain.

 

Hospital Internship Day-4 (06/05/2024)

It is a great day for us as hospital intern. Today we visited MRI and CT scan room. I was astonished seeing these two giant machines. Previously, I never ever visited such big machine. The hospital service manager Taizur Sir informed chief technician Jobayar sir about us. He welcomed us and talked with us, shared a lot of experiences. At first, I thought he is just like a common staff. But after talking with him a little, I could realize that this man is a gem for this hospital.

He told us how he came to this position. Paramedical is considered low level job here in our Bangladesh. But, with the suggestion of one of his senior sisters from his locality who was a medical student, he admitted into paramedical. At first, he was depressed. Then getting the real scenario from an MRI and CT scan technician of Square Hospital, he devoted himself into this profession. Now he is earning around 1 lack taka per month with only 6 hours duty per day.

MRI and CT scan are two most expensive diagnostic tests. The price of MRI machine is 20 crore and CT scan machine is 12 crore taka. The CT scan machine which can take picture within 14 second using 128 slice is one of the best in Cumilla city. The MRI machine contain 27 ton magnate.

I noticed the room decoration and lighting was different than others. They always maintain very low temperature so that these machines doesn’t face overheating issue. The monitors, computer system, printers, software everything seems costly. Even they have to hire experienced BUET engineer to fix any technical issue.

Patients with stroke, heomorhogic, road accident, spine cancer, brain disorder etc. come to this department. Although MRI considered as a safe process, CT scan has some side effects. Challenging things the technicians who work in CT scan room have to face are reproductive problem and blood destroy problems.

 

Hospital Internship Day-5 (07/05/2024)

Last couple of days, we visited different areas of Cumilla Trauma Center hospital. But today for the first time we meet with a doctor. At the beginning of this day, Taizur sir asked us our previous day’s experience. We expressed our gratitude. Then he directed us towards the emergency room doctor.

The doctor’s name was Nazmul Haque Siam. Our age gap is not much. He was may be five years senior than us. So, he could understand us very well. He explained us how he response when any patients come to emergency room. As this is a specialized artopedic hospital, most of the patients come here with road accident or other accidents. He provides them …… treatment. He notices whether the patients are in shock, pain, contaminated injury etc.

The doctor talk with us in a simple manner as like we are his younger siblings. We enjoyed it much. He was being honest with us. He said that they suggest antibiotic a lot to the patients with accident. He accepts the failure of controlling antibiotic use. But again, explained the overall environment is also not friendly to control this.

I was amazed when he recited different uses of different antibiotics and their importance. This made me curious to learn more about antibiotic. Believe it or not, this is for the first time I fell attraction towards pharmacology and human physiology-anatomy study. I think if the pharmacy students visit hospital seriously like us in his first or second year, he will be much more curious in his study.

One thing I did noticed. That is the promotional activity of pharmaceutical company by sending sample medicines and promotional materials. A lot of MPOs were there. They were promoting their company. This is not bad, but sometimes they use bad practices which is not good sign in Bangladeshi healthcare sector.

Usually, I submit the summary of our daily activities and experiences. But today, I had to go with my friend Dipu to visit another doctor in Gomati Hospital. So, I asked my groupmates to submit our summary. My friend Dipu talked with a doctor and convienced him to take an interview.


Hospital Internship Day-7, 8 (08/05/2024 and 09/05/2024)

Last two days we visited doctors chamber. Dr. Hassanat sir is one of the famous doctors here. A huge number of patients with asma, respiratory problem, allergy etc. come to this doctor. We observed some of his prescriptions and noticed some patterns.

He suggest influenza vaccine "InfluVax" to those who have not vaccinated yet. He also suggests different types of medicine in inhaler form so that the patients can get rid of their sufferings.

Till now, this hospital use traditional handwritten prescription which has many notable drawbacks. The main cause beyond this is maybe the senior doctors' unwillingness. We suggest, if possible, start applying electronic prescription with the juniors and young doctors. Then gradually it can be applied in the whole hospital. Surely, it will be a huge milestone for this hospital

At the peak hours, the number of patients in Hassanat sir's chamber increased in a number. It will be better, if possible, refer some patients to another doctors.


Hospital Internship Day-11 (15/05/2024)

Today, we completed an excellent session in the HR department and learned a lot. The session was conducted by Taizur sir, who has supervised us from the very beginning of our hospital internship. With his extensive career in this field, he has gained a wealth of experience. He shared some of it with us. The best part is most of the time he provided real life examples.

Before this session, I did not know the differences between the HR and Admin departments. I thought they were the same. However, I now understand the distinct roles and importance of these two departments. The HR department is responsible for hiring employees, training them properly, providing appropriate salaries, evaluating performance, ensuring accountability, promoting employees as a reward, increasing their salaries periodically, handle issues related to employee relations, and even firing them when necessary. Therefore, the people working in HR must be highly experienced. On the other hand, admin department focuses on the operational and logistical aspects of the organization.

Our educational journey is almost over, and soon we will start our jobs in various organizations. Consequently, we must submit our CVs to our preferred companies. As Taizur sir has this experience, we asked him many questions about this process. Fortunately, he received a CV during this session and explained the candidate’s mistakes.

Leadership and management skills are essential for running an organization. I noticed that Taizur sir has extraordinary insights into leadership and management. We also discussed office politics, a prevalent issue that can hinder an employee's career. He advised us on steps to protect ourselves from such dangers.

We also learned about the organizational structure, an essential aspect of any organization. Taizur sir explained the organizational hierarchy of this hospital, including the roles of the chairman, managing director, medical director, CEO, HR, and admin. We found it very interesting.

Finally, he explained how to maintain a work-life balance, which is essential for leading a happy life. Overall, as a hospital intern, I have learned a lot, and I believe my teammates have too.
